Terms To Know
Patient: is someone who is using their medical insurance or has been referred by a licensed healthcare professional for physical therapy rehabilitation or massage therapy services. Treatment will be billed at the insruance rates.
Wellness Patient: is someone who is relatively healthy and pain-free and is using their medical insurance for wellness services that are covered under their physical therapy or massage therapy benefits. Treatment will be billed at the insurance rates. Some conditions that would fit in this category are people who have body aches/tightness, insomnia, anxiety, high blood pressure, trauma, or just very high levels of daily stress.
Wellness Client: is someone who does not want to use their insurance or does not have health insurance and would like to receive treatments for general prevention to stay healthy. Treatment will be billed at the normal cash rates.
Members: is someone who would like to pay out of pocket for wellness services and lock in the introductory rates ($50 for a 60 minute session). There is no cost for membership. You only need to let us know that you are interested in becoming a member and we will add you to our list. The only requirement is that you need to commit to at least 1 wellness services per month. If you miss more than 2 consecutive months you will be removed from the list.
Physical Therapy evaluation $80 (regular)/$120 (long)
Physical Therapy treatment $145/hour
Advanced Massage Therapy $100/hour
*Please note that these prices are based on about a 20% cash disount off the insurance rates.
*Please note that appointment length can vary depending on your needs. A typical length of time is 60-90 minutes.
Wellness Rates For:
Students/Seniors/Veterans/Reserve & National Guard/Fire & Police For people who fit in one of these categories we offer a flat rate of $45 for a 60 minute session for any service. Sessions can be scheduled for 30, 45, 60, 75, 90 minutes or more. We will need proof that you qualify for this discount. Please provide the proper identification that proves you fit one of these categories. We consider a Senior someone who is 62 years of age or older.
Services may be covered in full or in part by your health insurance or employee benefit plans. Insurance rates are subject to the services provided. Please check your coverage carefully by asking the following questions about physical or massage therapy coverage or you can print a copy of the Insurance Intake Form to make sure you obtain all of the needed information regarding your therapy coverage:
Do I have physical/massage therapy insurance benefits?
Is Integrative Body, Inc. in or out-of-network for my insurance carrier?
What is my deductible and has it been met?
Is there a maximum amount of units that can be billed for any of the codes used? (as below)
Is there a maximum amount of units that can be billed in a day?
How many sessions per year does my health insurance cover?
Is approval required from my primary care physician?
Do I need authorization for treatment before I make an appointment?
If you need to find out a more detailed understanding for the cost of your treatment then you may ask your insurance company for reimbursement rate of these typically used billing codes:
CPT 97001-Physcial therapy evaluation.
CPT 97112-Neuromuscular reeducation of movement, balance, coordination, kinesthetic sense, posture, and/or proprioception for sitting or standing activities.
CPT 97140-Manual therapy.
CPT 97150-Therapeutic procedure(s), group (2 or more individuals).

We can bill any health plan that offers out-of-network billing.
Cost per unit of service:
1 unit of service=15 min.
Physical Therapy Evaluation-$100 (regular) $150 (long).
Physical Therapy- $45.32/per unit
Rolfing/Biofeedback therapy $31.25/per unit
*Please note that Rolfing will be billed to your insurance as manual therapy (billing code 97140). Rolfing is covered by your insurance plan if you have massage therapy benefits. Please verify if you have coverage before scheduling your appointment if you are planning to use your insurance.
Cash, check and all major credit cards accepted for payment.
If you do not show up for your scheduled appointment and you have not notified us at least 24 hours in advance, you will be required to pay a $75 no-show fee. This fee is not covered by your insurance and is the patient's responsibility. If you reschedule more than a day before your appointment, this fee does not apply.
Patients have a financial responsibility of paying in full co-payments and co-insurances. Co-payments are generally a set payment amount that is due by the patient for every physical therapy appointment. Co-insurance is the amount that the patient owes that is not covered by their insurance. At Integrative Body we do not bill the patient beyond what the maximum allowed amount of reimbursement coverage as determined by their insurance. Please note that there are some insurances that do not have a maximum allowed amount, therefore the patient would be responsible for the full amount that is not covered by their insurance plan.
Patients who are using insurance benefits to pay for their treatments have the responsibility to do their best to work toward functional treatment goals to minimize the over-utilization of health care dollars. All treatment must be medically justified therefore goals will be assessed at every session. Failure to comply with the recommendation of your therapist, therefore demonstrating lack of focus on improving functional outcomes, could result in termination of your therapy at your therapist discretion.
